การสังเคราะห์กรอบจริยธรรมและความรับผิดชอบของวิศวกรชีวการแพทย์ในยุคปัญญาประดิษฐ์สำหรับสถานพยาบาล

บทคัดย่อ

        การพัฒนาเทคโนโลยีปัญญาประดิษฐ์ (Artificial Intelligence: AI) และการวิเคราะห์ข้อมูลขนาดใหญ่ ได้เข้ามามีบทบาทสำคัญต่อการยกระดับระบบสาธารณสุข โดยเฉพาะการสนับสนุนการวินิจฉัยโรค การวิเคราะห์ข้อมูลทางการแพทย์ และการตัดสินใจทางคลินิก การนำเทคโนโลยีดังกล่าวมาใช้ในสถานพยาบาลก่อให้เกิดประเด็นท้าทายด้านจริยธรรม ความโปร่งใส ความเป็นธรรมของอัลกอริทึม การคุ้มครองข้อมูลส่วนบุคคล และ ความรับผิดชอบทางวิชาชีพ โดยเฉพาะวิศวกรชีวการแพทย์ซึ่งมีบทบาทสำคัญในการออกแบบ พัฒนา และกำกับดูแลระบบ AI ทางการแพทย์ บทความนี้มีวัตถุประสงค์เพื่อสังเคราะห์องค์ความรู้จากวรรณกรรมวิชาการ ทั้งในและต่างประเทศเกี่ยวกับจริยธรรม เทคโนโลยี และการกำกับดูแล AI ในระบบสุขภาพ เพื่อนำเสนอ กรอบแนวคิดเชิงจริยธรรมและความรับผิดชอบของวิศวกรชีวการแพทย์ในยุคปัญญาประดิษฐ์สำหรับบริบทสถานพยาบาลในประเทศไทย การศึกษานี้ใช้วิธีการทบทวนวรรณกรรมเชิงสังเคราะห์ จากงานวิจัย เอกสาร เชิงนโยบาย และกรอบมาตรฐานสากลที่เกี่ยวข้องกับ AI ทางการแพทย์ รวมถึงแนวคิดด้านชีวจริยธรรม ระบบสังคม–เทคนิค และแนวคิด Learning Health System ผลการสังเคราะห์พบว่าการประยุกต์ใช้ AI อย่างรับผิดชอบจำเป็นต้องอาศัยกลไกกำกับดูแลแบบหลายระดับ ทั้งด้านเทคนิค องค์กร และนโยบายสาธารณะ การพัฒนาสมรรถนะของบุคลากรและระบบธรรมาภิบาลข้อมูล จากการวิเคราะห์ได้เสนอกรอบแนวคิดเชิงปฏิบัติสำหรับสถานพยาบาลในประเทศไทยประกอบด้วย 7 องค์ประกอบหลัก ได้แก่ เสาหลักด้านจริยธรรม ความรับผิดชอบ ด้านเทคนิค การบูรณาการทางคลินิกและการกำกับโดยมนุษย์ ธรรมาภิบาลระดับองค์กร ความสอดคล้อง ด้านกฎหมาย การพัฒนาสมรรถนะบุคลากร และการติดตามระบบตลอดวงจรชีวิตของแบบจำลอง AI กรอบแนวคิดดังกล่าวสามารถใช้เป็นแนวทางสำหรับการกำหนดนโยบาย การกำกับดูแล และการพัฒนาเทคโนโลยี AI ทางการแพทย์อย่างมีความรับผิดชอบในบริบทของประเทศไทย
